I have read and agree to the policies listed here.
Expectations:
Employees are expected to report for all scheduled shifts on time and be ready to work.
Employees are expected to clock in no more than 10 minutes before their scheduled shift.
Notification:
Employees must notify their supervisor at least one hour before their scheduled shift start time if they will be absent or late.
Tardiness:
Tardiness is defined as arriving more than 10 minutes past the scheduled start time. Three instances of tardiness in a 30-day period will result in disciplinary action.
No-Show:
A no-show is an unexcused absence for a scheduled day of work without prior notification. A no-show will result in immediate termination.
Disciplinary Action:
1st occurrence: Verbal warning.
2nd occurrence: Written warning.
3rd occurrence: Suspension.
4th occurrence: Termination.
Arkansas is an at will employment state and severity of action may escalate the disciplinary action as needed.
Exceptions:
This policy does not apply to legally protected leave, such as Family Medical Leave (FMLA) or Americans with Disabilities Act (ADA) accommodations, which are handled separately.
Planned absences, such as vacations must be approved by supervisor within one week of planned time off.
The second time an employee is unable to come to work due to illness, they must provide a doctor's note upon returning to work.
Please provide the last ten years of employment and explain any gaps in working.
Thank you for submitting your application. If we believe that you are a good fit for one of our positions, we will reach out soon.
